The MOT/TTC Training Administration Program has standard Exam Score Bubble Sheets that you need to use when you proctor MOT/TTC intermediate, advanced and refresher exams. To download these blank templates, please login to your portal and download them from the Training Material section of the Provider Portal.

Step 1: Navigate towards the Provider Portal from the Program's main website.

Step 2: Login to the Provider Portal using your credentials.

Step 3: At the lower right corner, click on the Bubble Sheets tab to access the empty exam score sheets.

Step 4: Download the Exam Bubble Sheet and print copies to distributed to your students at your next MOT/TTC Exam.

Please note that the bubble sheets are also known as scantron score sheets.
